How to fill out the Five Order Form

Once you’ve opened the order form in spreadsheet editing software such as Microsoft Excel or OpenOffice, follow the five steps below to make sure you fill the form out correctly. Filling out the order form well ensures that you’ll get exactly what you want!

Step 1: Fill out your team info, contact info, and shipping address.

Step 2: Fill in your roster of players.

Step 3: Select the gear you’ll be purchasing and how many screens will be printed on each item.

Step 4: Fill in each player’s size, printed player number, and printed player name.

Step 5: Tell us the details about how you want your gear customized!

These five steps are outlined in greater detail below. For starters, the image below shows the general location of each of the five steps on the order form.

Step 1: Fill out your team info, contact info, and shipping address.

Filling all this info out accurately helps us contact you if we have any questions, complete your invoice accurately, and make sure that your gear gets sent to the right place!

Step 2: Fill in your roster of players.

While not required, this will help you keep track of which player is ordering what gear. Once all the gear specifications are entered on the form, you’ll also be able to see how much each player will be paying for his or her subset of gear.

This order form supports up to 40 players. If you are ordering gear for more than 40 players, use a second order form.

Step 3: Select the gear you’ll be purchasing and how many screens will be printed on each item.

Click on the item cell to bring up the drop-down arrow to the right of the cell. Click on the drop-down arrow and select the item you are ordering from the list. Next, click on the color cell to bring up the drop-down arrow to the right of that cell. Click on the drop-down arrow and select the color of the item. If you are ordering a color that isn’t available on the list, be sure to note the color in the notes section at the bottom of the order form.

This order form supports up to 4 items. If you are ordering more than 4 items, simply use a second order form.

Step 4: Fill in each player’s size, printed player number, and printed player name.

Since each row is designated for an individual player, fill out each player’s size, printed number preference, and printed name preference for each item he or she is ordering.

While every item will have the same level of logo customization, as many (or few) players as you wish may get printed numbers or names.

Step 5: Tell us the details about how you want your gear customized!

At the bottom of the order form give us the down-and-dirrrtay on how you want your gear to look. For each item, make sure you point out:

- the size of each printed logo/number/name.

- the location of each printed logo/number/name.

- the ink color(s) of each printed logo/number/name.
